Get a copy of a death certificate

A certified copy of a death certificate may be issued to:

  • The spouse, parent, child, or sibling of the deceased;
  • The lawful representative of the spouse, parent, or child of the deceased;
  • A person requiring the record for a documented medical need;
  • A municipal, state or federal agency when needed for official purposes;
  • A person with a New York State Court Order issued on a showing of necessity.

What is  lawful right or claim?

If the applicant is not the spouse, parent, child or sibling of the decedent, a lawful right or claim must be documented. An example of a lawful right or claim would be a death record needed by the applicant to claim a benefit. Documentation would consist of an official letter from the agency verifying that to process the claim they require from the applicant a copy of the requested death record.

There is a $10 fee for a certified copy of a death certificate.
