A certified copy of a death certificate may be issued to:
- The spouse, parent, child, or sibling of the deceased;
- The lawful representative of the spouse, parent, or child of the deceased;
- A person requiring the record for a documented medical need;
- A municipal, state or federal agency when needed for official purposes;
- A person with a New York State Court Order issued on a showing of necessity.
What is lawful right or claim?
If the applicant is not the spouse, parent, child or sibling of the decedent, a lawful right or claim must be documented. An example of a lawful right or claim would be a death record needed by the applicant to claim a benefit. Documentation would consist of an official letter from the agency verifying that to process the claim they require from the applicant a copy of the requested death record.
There is a $10 fee for a certified copy of a death certificate. We will also need a copy of your driver's license, if driver's license is unavailable, we will accept a photo identification.
